The attached photo is a new plant/weed in my yard. Can you identify it and tell me how to kill it and prevent it in the future.

It's hard to see enough detail, but it looks like a type of chickweed, which is a very common lawn and garden spring weed. Nuisance chickweeds are often winter annuals, and will die on their own by late spring or early summer; they germinated last autumn, overwintered, and flower to finish their life cycle in spring. You can either weed them out by hand or, if necessary, spot-treat them with an appropriate herbicide (such as a broadleaf weed killer for lawns). Their presence often indicates that a lawn is struggling to stay vigorous and dense (which tend to out-compete these types of weeds on its own), is mown too short, or that garden beds might need a topping-off of any mulch (aim for a 2- to 3-inch thick layer).
