For the 2nd season in a row something’s disrupts the roots about 3/4-1” below the soil surface as depicted in the attached photos. The plant begins to whither and and starts to tip.

Your green beans have a root rot. You will want to grow them in a new spot next year. See the link for what to do next year.

It is not too late to start new plants in another spot.
sadly, fungicides can’t cure fungal infections of plants, they can only prevent them.
