Dave - in reviewing your soil test I see that your soil is very healthy with nutrients at or above optimum. Per the recommendations, the fertilizer ratio is 3:0:3 which means you can purchase any product with equal parts Nitrogen and Potassium. Then come back to the MSU soil test site and enter the numbers into the Fertilizer Calculator (found under Tools) and the site will calculate for you the amount of that product to apply in one year over 1,000 sq. ft. Remember to pro-rate the amount if your area is lesser or more than 1,000 sq. ft.
Phosphorous easily fixes with the soil so there is little you can do to change that and the Magnesium will eventually be taken up by the plant roots and should not be an issue unless you see interveinal chlorosis in the leaves.
