There are several fungal diseases that cause similar looking lesions on blueberry leaves. I am leaning toward septoria leaf spot but it's difficult to tell exactly.

My recommendation would be to send in a plant sample to the UNH Plant Diagnostic Lab for analysis. This will give you positive confirmation and will allow you to take the appropriate steps.
