What does it mean when cones are only at the top of the pine tree?

It means the tree is normal. Conifers have separate female and male cones. This is an evolutionary trait to reduce self pollination. Male pollen cones are typically on the lower part of the tree and female cones are on the upper 1/3 of the trees. Conifers seeds/seedlings that are self pollinated usually won't develop or if they do are less vigorous.
