Red Maple

What would cause a 50 yr old Red Maple to die?

It is impossible to say what killed your maple. There could have been a combination of factors, but high on the list would be those related to cultural/environmental issues, as opposed to anything concerning insects or disease.
Hig on the list of these factors include girdling roots, drought, being planted or mulched too deeply, etc.
