Perennials falling over

Sometimes I find that my perennials are falling over, after they seemed to be growing so well. I’ve had this happen to yarrow, California poppy, Oregon sunshine. What am I doing wrong?

Without any information concerning the general care for these plants my best guesses include one or more of the following:
- Lack sufficient sunlight; all do well with all day sunlight;
- And/or excess water; all do well with no, or very little, supplemental water after they are established;
- And/or excess fertilizer; all do well on their own without supplemental fertilizer.
