The first thing you should do is positively identify that what you are dealing with are fleas and not some other type of insect or pest. Putting them in a bit of rubbing alcohol will kill them so that you can look at them under a microscope.

Assuming that they are fleas, they can be very frustrating to deal with, especially in warmer climates where they can live inside and outside all year long. The University of Tennessee has put together a nice informational booklet at this website: https://extension.tennessee.edu/publications/Documents/PB1596.pdf

The main thing to keep in mind is that you need to control them both on the dog (which the monthly meds should do) as well as in the environment. Fleas spend most of their life cycle off the dog, so washing the bedding, etc. is a good idea, as well as vacuuming well all the areas he has access to, and throwing out the bag or emptying the canister outside of the home immediately.

Depending on what types of flea meds your dog is on, there are a number of reasons why they might not be working. Some of them require that the flea bite the dog in order to be poisoned, so it is not unusual to see the occasional adult flea on a dog, because it just hasn't bitten the dog yet. Others are repellents and others control the reproduction of the fleas but don't kill the adult fleas. If this is a medication you've gotten from your veterinarian, you should be able to ask them to explain why it isn't working.

Good luck.
