Black Hills Spruce Problems

We have a Black Hills Spruce that we planted 18 years ago as it was supposed to be very disease resistant. Sadly, the last two years, it has been losing its inner needles and this year basically has only the current years growth. The tree is between two Balsams that seem to be doing just fine.

Is this age? Is this disease? Is it a pest?
