How do I care for a 70 year old rose bush?

I have an old rose bush approximately 60-70 years old. It is getting spindly. It is over 6 ft tall and I want to know if i should cut it way down this fall? What fertilizer should I use and when. It will break my heart if it dies now.

Hi,
