neurology and psychosis

I am writing a book and am looking for some advice on neurology and psychosis. My question relates to audio and visual hallucinations. What types of physical or psychological disorders would cause sustained audial and visual hallucinations? Is there a disorder that would also affect a persons ability to encode and understand spoken language? For reference I am writing a character who believes he lives in a word with demons that attack people and he is the only person who can fight them but it turns out he is having sustained hallucinations from birth.
