, we have a crepe myrtle that's about 15 ft tall that sprouted out of a stump of a crepe myrtle should we prune it because the trunk is only about the size of a silver dollar will it get thicker

Yes, this is typical of how crape myrtles grow. they grow tall then thicken each year.

Keep other thin branches cut back (called suckers)., unless you want a multi-stemmed tree which is preferred by many.
Keep 3-5 stems for best look in my opinion.
