What is causing my azalea to decline?

My azalea Molis has been in the same location for 25 years. It has done well with blossoms every year - some years more prolific than others. However, this spring it has several bare branches with few leaves and fewer blossoms. In these 25 years, I have never done anything to it - no pruning, no fertilizing. Does it need to be fertilized or transplanted?

It was most likely caused be or lack of snow cover last winter. We are getting many calls due to winter stress/kill.
