Can you identify this azalea problem?

The plant was stunted for a number of years and then began to thrive after tackling weevils with nematodes.

From the photo that you sent it does not look like the azalea was subject to an insect or disease issue. It looks like the foliage may have sustained some type of chemical damage. This may be a possibility if something was sprayed on the foliage or nearby.
Alll you can do is monitor the shrub throughout this growing season. You can send us additional photos if necessary.
