Why are my broccoli plants small and segmented rather than nice heads?

Broccoli plants form one larger head. After you pick that (you should break it off, not cut it), a ring of smaller heads will form. As you pick them, another ring will form, etc. Eventually your plant can be several feet wide and each ring gives you more but smaller broccoli clusters. It only produces one large head per season.

Misunderstood, There never was a large head, only small segmented heads. What causes this?
