Benefits under 5 different types of health insurance plans

When enrolling in medical insurance through your employer you receive a multi-page list of co pays, deductibles, maximum out of pocket, and all the different categories of medical service and their corresponding co pays and deductibles. Please provide this multi-page information for each of the plan levels: catastrophic, bronze, silver, gold, and platinum. Thank you.

To find out how much each plan level will cost, first go to healthcare.gov and select your state in the dropdown menu on the Home page. This will take you to your state enrollment site. There you will enter how many people and what types of coverage you are shopping for and the results will show different plan levels and costs. It is important to consider ALL costs such as deductibles, co-pays and coinsurance in addition to premium costs.
