100 year old Bartlett pear tree leaves have orange-brown spots. Is this evidence for a disease, insect damage or other blight?

Thank you for your question, Kelcey.  This a symptom of a fungus called pear trellis rust, described here:  https://pnwhandbooks.org/plantdisease/host-disease/pear-pyrus-spp-trellis-rust-european-pear-rust

Since none of the chemicals listed are available to homeowners, the only "cure" for this, as the article indicates, is to remove all juniper within a 1000-foot radius.  If this is not possible, you might want to contact a company licensed in applying a fungicide to help control the problem.  Good luck!
