Apple Cedar Rust #875346

I’m wondering what this is on my fruit tree and how to treat it.

I don't know if it is cedar apple rust since that species of rust is not too common in the Pacific Northwest, but it is one of the rusts caused by Gymnosporangium spp., and the behavior and management is similar. There are several species of rust in the Pacific Northwest that affect pears and/or apples (it is more common in pear). Rust fungi typically have two different hosts, the primary host (ex. your apple tree) which will display severe symptoms, and a secondary host (ex. a nearby cedar or juniper tree) which may only display minor symptoms and effects.

The best control is to identify and remove the alternate host(s) (cedar or juniper trees) that are within a 1,000-foot radius of the fruit tree, however, in urban environments this may not be an option if the hosts are in neighboring yards. There are no chemicals registered for home use, so your best option is to hire a company that is licensed to apply a commercial pesticide (see: http://npic.orst.edu/pest/selectpco.html). Other management practices include removing and destroying infected leaves, stems, and fruit to limit the spread; collecting and destroying fallen fruit beneath trees; avoiding overhead sprinklers or irrigation that wet the leaves; and of course, removing alternate hosts when practical.
