The tree appears to be a crabapple. The appearance of the leaf and the early summer leaf loss suggest the tree may be suffering from apple scab. The apple scab fungus has a life cycle that involves overwintering in fallen leaves, spore release during spring rains, infection of developing leaves and fruit, and repeated cycles throughout the growing season. Warm, rainy weather creates ideal conditions for apple scab. Good cultural practices can help reduce the chance of infection. Control includes getting rid of dropped leaves and appropriately timed fungicides. Check out the following links for more info:
