We have an apple tree with already diseased looking baby apples and the trunk looks like black rot - possibly over 200 apples this year and when looking over half have blemished or holes etc - wondering how to treat? Should we remove the blemished ones so the healthy ones can grow better? Should we spray? Maybe it isn’t black rot? You can zoom in and see black the farther up you go on trunk picture

As you are suspecting, the likely culprit here is black rot. It is primarily managed by good sanitation and pruning out of any affected branches. You might also consider a fungicide, which would be applied in the spring. Inspect your apples carefully, remove the damaged ones, and eat only those without blemishes.
