Apple tree damage #869241

I am concerned about the health of my apple tree. It has a large bark scab (?) and the leaves on one side are yellowing. I am hoping someone can let me know what might be going on and how to help the tree.

Cytospora cankers are affect many different trees and apple is one them. They are opportunistic and usually damage trees that are stressed by moisture extremes or physical injury. It appears that your tree is infected and, unfortunately, the main infection appears to be located at the junction of multiple trunks which prevents you from pruning out the infection. In my opinion, this tree is not going to make it however you might want to have an arborist do an on-site examination. The arborist will be able to see more than we can in a photo and will be able to give you management recommendations.
